Long underwear, also called long johns or thermal underwear, is a style of two-piece underwear with long legs and long sleeves that is normally worn during cold weather. It is commonly worn by people under their clothes in cold countries.   In the United States, it is usually made from a cotton or cotton-polyester-blend fabric with a box-weave texture, although some varieties are also made from flannel, particularly the union suit, while many newer varieties are made from polyester.  Other fabrics can also be utilized when manufacturing long underwear.  A union suit is a type of one-piece long underwear. Created in Utica, New York, United States, it originated as women's wear during the 19th-century United States clothing reform efforts, as an alternative to constricting garments, and soon gained popularity among men as well. The first union suit was patented in 1868 as "emancipation union under flannel".  Traditionally made of red flannel with long arms and long legs, it buttoned up the front and had a button-up flap in the rear covering the buttocks (colloquially known as the "access hatch", "drop seat", "fireman's flap", and other names), allowing the wearer to defecate without removing the garment.

European manufacturers use wool blends or even 100% wool, usually Merino or other high-quality wool. Some models might include a thin layer of polyester to transport moisture away from the skin. Wool, in addition to being fire retardant, provides highly effective insulation and will keep its insulating properties even when wet, as opposed to cotton. In central Europe in Poland there is still one company called EWAX, which manufactures traditional cotton Long Johns.

Long underwear is often referred to as "long johhs".  Long johns were first introduced into England in the 17th century, but they did not become popular as sleepwear until the 18th century. They were supposedly called long johns after a famous knife fighter who fought in long underwear. They were first used as loungewear but then later became popular as sleepwear. Children also wore long johns as sleepwear, usually made out of cotton and polyester.
