Turn a fit session into clear, trackable changes your factory can act on.
After a fit session you need to tell the factory exactly what to change. This form has two parts. A measurement table compares your spec with the actual sample, calculates the variance and tells you how far to move each measurement to match the spec. A fit observations table records each issue with its area, a priority, a status and a photo reference, and counts open items. It ends with a decision and the due date for the next sample. The printable PDF works at the fitting table.
This form records changes to send to the factory. For a pass or fail review of a sample, use the Sample Approval Checklist.
Enter the style, round, size, date, who reviewed it and the fit model or form with their measurements. Choose inches or centimeters, since the form uses that unit in its advice. Using the same model or form each round makes results comparable.
For each point of measure, type the spec, the actual measurement on the sample and the tolerance. The variance is actual minus spec, so a positive number means the sample is larger than the spec. The next column says Within tolerance, or how much to reduce or increase to match the spec.
For anything that does not look or feel right, add a row: the area, its priority (High, Medium or Low), a status, a photo reference, and a specific request, such as lower the neckline by 1/4 in. Specific, measurable requests are fixed quickly, and vague ones come back wrong.
Set the status to Done as the factory fixes each item in the next sample. The form counts open items and open high-priority items, so you can see what still needs attention.
Choose Approved, Approved with changes or Revise and resubmit, set the due date for the next sample and sign. Send the form with photos to the factory the same day, and update your tech pack and its version.

It is the actual measurement minus the spec. A positive result means the sample is larger than the spec, and a negative result means it is smaller.
It shows how far to move the sample measurement to reach the spec, for example Reduce by 0.75 in. If you want a different target, say so in the comment column.
The checklist is a pass or fail review of a sample. The fit comment form records the specific changes you want, with priorities, to send to the factory.
